PERSONAL DOCUMENTS

•  Accomplished application form
• Copy of your passport  (current and old)
•  Photo page of your passport and any pages with amendments, endorsements, visas and entry / exit stamps.
• 1 passports sized photo attached to the front of the application form
• NSO/ BIRTH CERTIFICATE
• MARRIAGE CERTIFICATE (if married)

 
FINANCIAL DOCUMENTS
Proof of funds (cash, travellers’ cheques, bank statement or recognized credit cards with sufficient credit available)

• Copies of your bank statements/ deposit books showing your history of savings
• Copy of bank certificate
• Copies of pay slips over a period of time 
• Credit card statements and limits 

 
ADDITIONAL REQUIREMENTS
Evidence of current employment or documents supporting business ownership

• COE
• Employment contract
• Travel authority (Gov’t office worker)
• ITR/  Certificate of Compensation Payment / Tax Witheld (BIR Form 2316)

 
IF SELF EMPLOYED// WITH BUSINESS

• BIR
• DTI/ SEC
• Mayor’s Permit

 
SUPPORTING DOCUMENTS

• Evidence of intended purpose of travel
• Itinerary
• Medical certificate (if staying at least 6 months)
• NBI/Police Clearance (if staying for a total of 24 months or longer)
• Declaration by an acceptable sponsor or letter of invitation from friend or family based in New Zealand
• Personal photos with the acceptable sponsor
