TOURIST WITHOUT GUARANTOR (visit friends or distant relatives)

1.      PHILIPPINE PASSPORT  (Broken Lamination of the Photo part is not accepted and valid within six 6 months)
2.      VISA APPLICATION FORM printed in A4 sized
3.      PHOTO 2X2  WHITE BACKGROUND must be pasted on the application form
4.      BIRTH CERTIFICATE (must be from PSA main office issued within one 1 year)with attached Official receipt
              IN CASE OF NON-RECORD OF REGISTRATION OF LIVE BIRTH
                1.   Non-Record in the PSA. Need certificate of Non-Record from the PSA and
                       BIRTH CERTIFICATE from the LOCAL CIVIL REGISTRAR.
                 2.  Non-Record in the PSA and LOCAL CIVIL REGISTRAR. Need certificate of
                       Non-Record from the PSA and the LOCAL CIVIL REGISTRAR.
              IF PSA ISSUED RECORD IS NOT CLEAR
                1.    BIRTH CERTIFICATE from the LOCAL CIVIL REGISTRAR.
              IF BIRTH CERTIFICATE IS LATE-REGISTRATION
                1.   Birth Certificate from the LOCAL CIVIL REGISTRAR.
                2.   Original Baptismal Certificate, School Record (Form 137 or report card) School Year Book.
                      (Need contact tel # of the church, and contact tel# of the school)
5.      MARRIAGE CONTRACT ( if married)
            (Must be from PSA main office issued within one 1 year)
            Exemption of birth certificate and marriage contract if the applicant has his/her passport (OLD OR VALID)                    with a used Japanese visa
6.      DAILY SCHEDULE IN JAPAN (TAIZAI NITTEIHYO)
7.      BANK CERTIFICATE (must be issued within 3 months)
8.      INCOME TAX RETURN- original and photocpy (FORM 2316 in BIR)
9.    CERTIFICATE OF EMPLOYMENT (stating position, tenure of service and with
          Gross annual income for Employees, must be one (1) month valid)
10.     Certificate of Business Registration or SEC Registration, DTI and
          Mayors Permits– for self employed

TOURIST WITH GUARANTOR (visit relatives)

1.      PHILIPPINE PASSPORT  (Broken Lamination of the Photo part is not accepted and valid within six 6 months)
2.      VISA APPLICATION FORM
3.      PHOTO 2X2  WHITE BACKGROUND must be pasted on the application form
4.      BIRTH CERTIFICATE (to prove relationship)
5.      MARRIAGE CONTRACT ( if married)
            (Must be from NSO main office issued within one 1 year)
            Exemption of birth certificate and marriage contract if the applicant has his/her passport (OLD OR VALID)                   with a used Japanese visa
6.      DAILY SCHEDULE IN JAPAN (TAIZAI NITTEIHYO)
7.      BANK CERTIFICATE (must be issued within 3 months)
8.      BANK STATEMENT
9.      INCOME TAX RETURN- COPY (FORM 2316 in BIR)
10.    INVITATION LETTER from Guarantor in Japan (Must have handwritten signature)
11.    FAMILY REGISTRATION (KOSEKI TOHON)
12.    RESIDENCE CERTIFICATE (JUMINHYO)
          (10,11 is only applicable if Guarantor is a Japanese)
           Note: 10 and 11 must be issued withint 3 three months
13.    A certificate of residence (JUMINHYO) with description of his/her family relationship with all Family                       members from City Hall Passport Copy (all pages) or a copy of Residence Card. (12 is only applicable if               Relatives in Japan is not Japanese)

Note: for cases wherein the guarantor is the relative in Japan, a certificate of residence (JUMINHYO) from city hall (issued within three months) is necessary)
